Required Promotional Media AuthorizationI specifically authorize the photography, filming, recording, editing, reproduction, publication, display, distribution, transmission, posting, archiving, and other use of the Student’s name, image, likeness, voice, appearance, participation, performance, student work, projects, interviews, testimonials, and related media for the purposes described in the Packet. Binding Individual Arbitration, Jury Trial Waiver, and Class Action WaiverI understand and agree that covered disputes will be resolved through binding individual arbitration in California under the terms of the Packet, except for qualifying individual small-claims matters or other relief preserved by the Packet or applicable law.
